Well I don't see why the "email" field would not appear...
Did you create a customized view of our module in your template?

Did you somehow altered our code which could explain that issue?

If not, could you please re-download AcyMailing from our website and install it again just to make sure you're using the latest stable version...

And if it still does not fix the issue then I will need a temporary access to your website + FTP to have a look at it and determine what's going on.

The e-mail field is added but hidden by some CSS.

In your own template CSS file templates/theme817/css/layout.css on line 2234 I can see:

.acyfield_email input {
    background: none repeat scroll 0 0 transparent;
    border: medium none;
    color: #FFFFFF;
    font-weight: bold;
    margin: 6px 0 0 15px;
    width: 280px;
}

This code hides the e-mail field on your template... (white text field with no border...)
Please remove this code or change it (border and color)
